If head lice are detected on a student, the school should provide parents/guardians/carers with an 'Action Taken form' to gain an understanding of the treatment used and when it commenced. This form should be attached to the Department of Health (DH) " Treating and Controlling Head Lice "pamphlet. This pamphlet can be downloaded from the DH website Head Lice in PDF format, translated into 21 languages.
This form should also be attached to the Alert Notice, so as parents who find head lice in their child's hair are able to inform the school of what treatment has been used and when it commenced.
This form should be handed in to the Principal when the child returns to school.
