Children and young people undertake a number of important transitions at key points during their education; moving from kindergarten to school, primary school to secondary school, and finally leaving school to pursue post-school options.
Each transition point is a significant milestone to celebrate and can be both exciting and daunting. For children and young people with disabilities and their families, there may be additional challenges and uncertainty at these times. It is important to seek information, support and advice ahead of each transition point.
A range of customised resources are available to support children and young people with disabilities as they undertake these important transitions.

Transition from Kindergarten to School

An enhanced process supporting the transition to school for children with severe disabilities in receipt of a Kindergarten Inclusion Support package.
The Process includes:
a) 'Sharing Our Journey' Protocol (pdf - 281.19kb) for early childhood services, early intervention services, preschool field officers, and schools to assist in implementing a plan to support transition for these children and families.
b) ‘Sharing Our Journey’ Information Kit (pdf - 910.68kb) for parents including information sheets, a child profile and an actions planner to support the child’s move from kindergarten to school.

Transition from Year 6 to Year 7

Transition from primary to secondary school is a process of change that for some students may require substantial preparation, planning, adjustment and support. Parents, Carers or Guardians support for students in years 6 and 7 provide information and advice for parents in supporting their children through this important transition.

Transition from School to Post-School Options

Strengthened Pathways Planning aims to foster independence and increase the participation of young people with disabilities in further education, training and post-school employment, through strengthened careers and transition planning in schools.
