School Curriculum Survey (Special Settings) for PSD students

The following MS Excel file has been developed to enable the entry of assessments for students on the Program for Students with Disabilities against the goals outlines in their Individual Education Program. 

• Data entry file for the School Curriculum Survey (Special Settings) for PSD students (Excel - 73Kb) (Note: please download this MS Excel file to your computer before entering data into the file)
• Measuring academic progress against each KLA (PDF - 836Kb) - outlines the framework for setting goals and measuring progress for PSD students

Once data entry is complete, summary data needs to be entered into the School Level Report webforms by following the instructions on the first sheet of the file. See School Level Report webforms (principal's edumail login and password required)
