Loss or Stolen - Loss Claims Process

Lost or stolen notebooks must be reported immediately to the police. The police will provide you with a police report that is to be submitted when making an insurance claim. The following forms are required to be completed:

Payment by credit card or cheque/money order is required before claim can be processed.

Completed forms, along with a $200 excess payment and police report are to be sent to Echelon Claim Services:

Once the claim form is complete, a replacement notebook will be issued to the recipient. If the recipient wishes to withdraw from the program, an accompanying cover letter must be sent with the claim forms indicating this.

Notebook contribution payments will continue to be deducted during a claim process, unless the recipient officially withdraws from the program. If a replacement notebook is not received within four weeks of the insurance claim being completed, overpayments will be reimbursed. Please contact the Notebooks team if this occurs: See Contact us

To reduce the risk of the Notebook being lost or stolen it is advised that a security cable lock is purchased. To purchase these locks, see ICT Products & Services Schedules.

Accidental Damage

Accidental damage to a Lenovo Notebook

Contact the Lenovo helpdesk on 1800 006 066 who will assess the notebook and repair it if it is repairable. An Insurance Excess Payment (damages) form must be completed and forwarded to the Notebooks Team along with payment to:

Accidental damage to an Apple MacBook

Please contact the Department’s insurance broker (Echelon Claim Services) on 9860 3461.

The insurance broker will determine the closest/most convenient repair location for the recipient and email the centre to notify them that a DoE repair will be sent to them. The email will authorise the repair centre to examine the notebook and provide a repair quote

The notebook recipient will be required to drop their computer into the repair centre where it will be assessed and a repair quote issued to the insurance broker. The insurance broker will authorise the repairs on behalf of DEECD.

The recipient must complete an excess payment form for damages and pay a $100   excess payment payable to DEECD

 

Uneconomical to repair

If it is deemed uneconomical to repair/replace. Apple/Lenovo may issue you with a quote. You must then send this to Echelon Claim Services along with the following forms.
