Registrations

Victorian Years 3 & 4 students (Government and non-government schools)

To take advantage of the free membership for Years 3 & 4 students:

1. Distribute and collect signed Parent Consent Forms for all Years 3 & 4 students registering for SuperClubsPLUS Australia. Parent Consent Form (Word - 33Kb)
2. Enter the details of participating students and teachers on the Student Registration Form (Excel - 67Kb)
3. Download a hard copy of the Principal’s Approval Form for the principal to sign, to give approval for the school’s participation and to verify teachers and students. Principal’s Approval Form (Word - 168Kb)
4. Scan the signed version of the Principal’s Approval Form and email it along with the Student Registration Form to the Department of Education & Early Childhood Development at superclubs.plus@edumail.vic.gov.au
5. If you cannot provide a scanned copy of the Principal’s Approval Form, you may fax a copy to (03) 9637 2150. Only electronic versions of the Student Registration Form will be accepted.
6. Store hard copies of the Parent Consent Forms at the school.
7. SCPA will send usernames and passwords for the registered teachers and students to the first contact teacher identified on the Student Registration Form for distribution at the school.
