Consent Forms

Schools wishing to organise head lice inspections must obtain written parental consent. When a student enrols at a school, parents/guardians/carers should be asked to provide written consent for the school to conduct head lice inspections, to cover the duration of the child's schooling at a particular school.

Written consent should be obtained by using the pro forma Consent Form to Conduct Head Lice Inspections (Word - 25Kb).

The consent form has also been translated into 21 languages.
