Action Taken Forms

If head lice are detected on a student, the school should provide parents/guardians/carers with an ' action taken form 'to gain an understanding of the treatment used and when it commenced. This action taken form should be attached to the Department of Human Services (DHS) " Treating and Controlling Head Lice "pamphlet. A copy of this pamphlet was provided to schools in the DHS " Scratching for Answers "head lice information kit. This pamphlet can be downloaded from the DHS web site for Head Lice (http://www.health.vic.gov.au/headlice) in PDF format, translated into 21 languages.

This form should also be attached to the Alert Notice , so as parents who find head lice in their child's hair are able to inform the school of what treatment has been used and when it commenced.

This form should be handed in to the Principal when the child returns to school.
