Pages with forms that collect personal information must have an explicit privacy statement

IPP 5.1 Openness

The location of privacy statements need to obvious to the viewer of a web page. While a link to complete statement covering all relevant aspects of privacy can appear in a common area on each page, where specific collection of personal information takes place, such as with forms or transactions, special attention should be given to the privacy aspects of this collection.

Requirement

All pages that present a form for collecting personal information must have an explicit statement accompanying the form regarding privacy issues of the information collected.

Checking Tool

In a browser view all forms in the web page to determine if they are collecting personal information. If so, ensure that a statement exists on the page that focuses on the privacy issues of the data being collected.
